Driver’s Licence Medical Assessments

If you’ve received a notice from NZTA advising that you need a medical assessment to renew your driver’s licence, we’re here to help.

Many drivers aged 75 and over are required to have a medical assessment as part of the licence renewal process. NZTA generally sends these notices around six months before your licence expires.

We recommend booking your appointment as soon as you receive your notice. This allows plenty of time for your assessment to be completed and any required paperwork to be submitted before your licence expires.

Driver’s licence appointments cannot be booked through the Health365 portal and must be made directly with Reception.

To ensure continuity of care, all driver’s licence medical assessments are completed by your registered doctor.

Before Your Appointment

When booking, please let reception know that your appointment is for a driver’s licence medical assessment.

Please note that your doctor will only have sufficient time during this appointment to complete your driver’s licence assessment. If you would like to discuss other health concerns, we recommend booking a longer appointment.

What to Expect

Step 1: Call reception to book an appointment with your registered doctor.

Step 2: When you arrive at the clinic, please ask reception for a DL9 form.

Step 3: You will first see one of our nurses for pre-assessment checks. Please note that this appointment incurs a separate nursing fee.

Step 4: You will then see your registered doctor, who will complete your driver’s licence medical assessment.
