Careplus is a funded programme for patients with long-term conditions who need more frequent appointments or follow-ups.

Funding is applied for by your GP/NP or practice nurse. There is a limited number of funded appointments available, and you will need to meet certain eligibility criteria.

Funding lasts for 12 months and is not automatically renewed. Other appointments not covered by the Careplus programme will incur a charge.
